Skilled Nurses routinely provide a comprehensive range of services in the comfort of your home:
- Skilled Nursing Services
Skilled nurses provide treatment prescribed by the patient’s health care team, assess changes in the patient’s status and work closely with the patient’s care providers to optimize healing. With an emphasis on patient and family education at their core, our nurses will help you safely manage at-home care and facilitate as much self-care for your loved one as possible.
- Physical Therapy
The physical therapist helps the patient regain strength and coordination. Services include therapeutic exercises such as range of motion exercise, mobility rehab, gait training.
- Occupational Therapy
The occupational therapist provides activities and exercises designed to help the patient resume self-care and normal daily life activities such as gross and fine motor functions of upper body and safety with bathing.
- Speech Therapy
The speech pathologist assists patients with impaired communication skills to regain those skills or to learn alternative ways to express their needs and feelings.
- Medical Social Services
Medical Social Workers help patients, and their families cope with personal and emotional impact of illness or disability. The social worker will also help locate financial resources and community assistance such as home meal delivery, support groups, transportation, or governmental support.
- Certified Home Health Aides
Home health aides are available to help with the patient’s personal care and hygiene needs and to provide light homemaking services.
- Congestive Heart Failure (CHF) Program for High-Risk Patients
Designed to increase the patient’s knowledge regarding heart failure, the CHF program improves our patients’ quality of life and helps reduce re-hospitalizations. Our program is ideal for patients who have been diagnosed with CHF, have recently been discharged from a hospital or a rehabilitation facility with a diagnosis of CHF, or are prone to acute and sudden exacerbations of CHF.
- Pneumonia Home Care Program
Managing patients with pneumonia at home can be less challenging when you have the help of our Pneumonia Home Care program. Our program helps you decide when you can handle the symptoms yourself and when you need our help.
- Chronic Obstructive Pulmonary Disease (COPD) Home Care Program
Patients with COPD have many needs when they are at home. Our staff can help you understand how to manage their care safely, as well as provide you a guide for when you need additional assistance. Our COPD Home Care program tells you what you need to know when you are caring for your loved one at home.
